Disability South West (DSW) is seeking a dedicated and motivated Support Coordinator to join our growing team in a full-time position.
About the Role
As a Support Coordinator, you will work closely with NDIS participants, their families, support networks, service providers, and allied health professionals to ensure participants receive the supports they need to achieve their goals and maximise their NDIS plans.

Key Responsibilities
Support participants/families to understand, implement, and maximise their NDIS plans to achieve their goals and improve their quality of life.
Coordinate supports and services, ensuring participants receive appropriate, timely, and effective assistance aligned with their NDIS plan goals.
Connect participants and their families to mainstream, community, health, education, housing, employment, and other relevant supports to promote inclusion and independence.
Build the capacity of participants, families, and carers through information, guidance, skill development, and support planning.
Develop and maintain positive working relationships with participants, families, nominees, service providers, therapists, and other key stakeholders.
Advocate for participants and assist them in navigating the NDIS, mainstream services, and broader support systems.
Monitor service delivery, identify gaps in supports, and facilitate appropriate referrals and interventions to achieve participant outcomes.
Maintain accurate case notes, records, reports, and documentation in accordance with organisational, NDIS, and legislative requirements.
Participate in meetings, reviews, case conferences, and planning discussions to support participant outcomes.
Identify opportunities to improve participant outcomes, enhance service delivery, and support continuous improvement initiatives.
Work within service delivery targets, performance expectations, and Key Performance Indicators (KPIs), ensuring efficient and effective support coordination service delivery.
Uphold participant rights, choice and control, and person-centred practices in all aspects of service delivery.

About You
To be successful in this role, you will have:
Minimum 2 years experience working within the disability sector
Strong problem-solving and decision-making skills
Excellent verbal and written communication skills
High level of attention to detail
Ability to effectively manage multiple tasks and competing priorities
Ability to work independently while contributing positively within a team environment
Commitment to participant-centred practice and achieving positive outcomes
