Scope of the Role

• Policy & Compliance: Work within policies, procedures, and guidelines established by Fusion Australia to develop and implement a person-centred Case Management Plan for each young person entering this casework program.
• Team Mentorship: Work to supervise and support and mentor youth team staff and volunteers to facilitate the effective implementation of a Case Management Plan for each client.
• Safe & Empowering Environment: Work as part of the team to provide a safe environment within the program that supports young people to take responsibility for their own lives and develop skills, awareness, and character.

About the Role

As a Youth Case Worker, you will deliver intensive, individualised casework for young people requiring targeted support. While our Youth Workers facilitate school-based programs and general community outreach, you will step in to provide short-term (up to 3 months) intensive case work intervention.

In this role, you will receive referrals from internal Youth Workers and Families Workers, local schools, and external services. A key aspect of the position is utilising our existing community outreach spaces as a "soft entry" point to engage vulnerable young people, build initial rapport, and transition them into formal casework.

Crucially, this role balances face-to-face support with the administrative side of casework. We are looking for an organised professional who is highly skilled in client management, data entry, and case note compliance to ensure our contract reporting requirements are met. You will operate independently to manage a focused average caseload of 12 clients at any one time (approx. 48 clients per year), whilst working collaboratively alongside our broader Youth and Families teams.

Brief Description of Role:

38 Hours per week - Monday to Friday (9am-5pm)

Responsibilities:

• Managing caseload of 12 young people at a time.
• Receiving referrals and assessing for suitability.
• Creation of initial plan that outlines key goals, tasks and actions and working to reduce barriers for young people to reach these goals.
• Case noting.
• Researching possible interventions and strategies.
• Providing information, referral and support.
• Providing psychosocial education.
• Keeping case planning tools and other resources up to date.
• Exit planning.
• Liaising with young person’s parents, school and other supports.
• Case coordination.
• Referral to specialised services.
• Ensure up to date case files and documents.
• Participating in the further development and review of the program including contributing to policy and risk management.

STANDARDS OF PERFORMANCE

• Provide timely and sufficient reports, Case Management plans and other records
• Maintain documentation and correspondence to a high standard in compliance with Fusion Policy and Procedures
• Fusion’s Policies and Procedures are followed and explained to clients and thier families.
• Respond to phone messages and emails promptly
• Be open to receive feedback from line manager and colleagues.
• Be willing to give feedback to line manager and colleagues.
• Model Fusion’s values in all interactions within your role
• Maintain professional relationships with support professionals and Fusion supporters
• Set and maintain appropriate boundaries with young people

SELECTION CRITERIA

Essential

• Qualification: A minimum of a Diploma of Community Services, Case Management, Youth Work (or a related discipline), or a bachelor’s degree in social work, Psychology, or Social Science.
• Demonstrated Experience: Proven experience delivering casework, case management, or intake and assessment within a community services environment.
• Sector Knowledge: Practical understanding of trauma-informed care and risk assessments.
• Experience providing person-centred, strengths-based, trauma-informed casework to young people (with an understanding of working within a Theory of Change model highly regarded).
• Understanding of common issues experienced by at-risk young people.
• Proven experience building engagement and rapport with young people (including utilising outreach as a soft entry to engagement).
• Excellent time management and ability to work independently.
• Sound understanding of case notes, documentation compliance, and client management databases.
• Understanding of the service system and referral pathways in NSW.
• A genuine passion for the Western Sydney community and supporting local vulnerable youth.
• A valid NSW Driver’s Licence, current Working with Children Check (WWCC), and National Police Check.
• Willingness to work from a framework of Christian values.
