About the Role:
South West Care Services Pty Ltd is a registered NDIS Service Provider. We specialise in providing in-home Care and Community Access to our Aged and Disability participants throughout Central Sydney and Greater Sydney.
As a community worker you will work with empathy to complete a variety of domestic, social, respite, and personal care services to ensure a quality life with dignity, respect, and social inclusion.

Duties and Responsibilities:
· In-depth understanding of NDIS support items as outlined in the NDIS Guidelines.
· Evaluate, design, and implement client support plans while ensuring adherence to South West Care’s policies and procedures.
· Identify and assess community needs, including healthcare, welfare, housing, employment, training, and other essential services.
· Build and maintain strong working relationships with community organizations, clinical services, and other relevant providers to facilitate effective communication, referrals, and coordinated care.
· Provide individual and group support to clients, helping them address social, emotional, and financial challenges while empowering them to make informed decisions and work towards their goals.
· Monitoring and reporting of clients' progress, including participant referrals, progress against NDIS Goal Plans, and data collection and reporting where required.
· Advocate on behalf of clients to access services within the community supporting our clients’ families by providing education and care.
· Preparing submissions for fundraising and reports to government bodies and other agencies.
· Undertake ongoing professional development as required.
In addition to the above responsibilities, you must also perform any other reasonable duties as directed occasionally.
Role Requirement:
· Qualification in Individual Support, Disability, Community Services or Aged Care.
· Current NSW driver’s licence
· Current First Aid Certificate
· Current Working with Children Check (or willing to obtain)
· Police Check (or willing to obtain)
· NDIS Workers Check
Skills and Qualifications:
· AQF Associate Degree, Advanced Diploma or Diploma in a highly relevant field
· At least 2-3 years of experience in a relevant position.
