Community Support Partner - Narrabeen

About the role: 
Our Seniors Team currently provides low level Flexible Respite services under the Commonwealth Home Support program.  We are a growing business and are looking for dedicated Support Partners to grow with us on our journey towards Support at Home.    
  
Our Community Support Partners use a person-centred approach to assist our aged care clients to live independently within their own homes and communities.  Our goal is to ensure client dignity, choice and to be able to live happier and healthier lives. 
  
Location: Narrabeen area  
Classification: Casual or part time 
Days and hours: Tuesday, Wednesday and Friday mornings from 7am to 9am and in the afternoons on Monday, Tuesday and Wednesday from 3pm to 5pm.  
Rate: SCHADS Award (Home Care) Level 3 and the KMs are reimburse for travel incurred between client homes. 
  
What your days will look like: 

• Achieve Goals Together: Implement individual care plans to help participants reach their goals. 
• Care with Heart: Provide person centred care and ensure that every client is living with dignity, compassion, and respect. 
• Community Connection: Assist with community participation and foster meaningful social interactions. 
• Stay Informed: Know participants' profiles what they can and cannot do and always being aware of the unknown.   
• Positive Engagement: Interact positively with participants, families, carers, stakeholders, and team members.   

About you: 

• Current unrestricted NSW driver’s license and access to an insured vehicle. 
• Cert III Individual Support (Ageing)  
• A commitment to following high standards of care. 
• Current Senior First Aid. 
• Ability to travel, Km’s paid on travel between clients.
