Right at Home Gosford is seeking a Care Partner to join our local team and play a crucial role in providing high-quality in-home care services to our valued clients. If you have a strong background in aged care and community services, excellent communication skills, and a commitment to enhancing the well-being of seniors and vulnerable individuals, this opportunity is for you.

Your new role will be responsible for:
Meeting with clients and their families, reviewing their welfare and building strong connections
Implementing and reviewing care plans to best meet client requirements
Handling information requests, feedback, and general enquiries across a variety of stakeholders
Ensuring all documentation is accurately and comprehensively completed in Right at Home databases and software programs
To be successful in this role, you will need:
Relevant qualification in Aged or Community Care and experience with care coordination
Highly developed verbal and written communication skills
Strong organisational, problem solving and time-management skills
Proficient computing skills
Ability to manage client funding budgets
Current Australian National Police Check and First Aid & CPR certificate
Sound understanding of the Aged Care Support at Home funding model
Demonstrated experience supporting care staff
